Paperwork! Yeah!
Unfortunately, forms are a necessary evil, but at least we have them handy here for you to download and print out.
We currently have:
- General Permission Form
- General purpose consent/liability form to certify that it really is ok for you (or your child, depending on who you are) to do a given activity with the youth group. Usually needed for activities not at the church.
